I have the same problem - bought an iPhone at an Apple store being told I was only required to keep the data package for 3 months and then after 3 months when I called to remove it was told that was incorrect. Now I can't even use my iPhone as a phone - had to get another one, and my husband is using the iPhone as an iPod! I just wanted to mention the FCC form was PDF, so I had to create a Word doc so that I could email the form - the PDF form won't allow you to enter the fields and then email it. Just wondered if you received much response from this.
